Login changed at startup

Hey all,

My father is running my old imac 2007 model. Cant remember the details of the hardware.

He called me and said that his user account was removed completely and now there is a user he didn't create. This new account is the admin along with a password he didn't create.

Was he hacked? How should we fix this?

I'm guessing hacked... boot from the original CD and do a clean install.
