I have tried to set 2 apps to start at login, Skype & Desktop monitor.
I have used the following:

"Mac OS X 10.3 or later
Log in as the user who wishes to have a login item or as an admin user.
Choose System Preferences from the Apple menu.
Choose Accounts from the View menu.
Click the name of the user.
Mac OS X 10.3.x: Click the Startup Items button.
Mac OS X 10.4 or later: Click the Login Items button.
Click the "Add {+}" button.
The item you selected will automatically open the next time you log in."

The items show when I enter them with the + as above OR right click in the dock & select "options" then "Open at login".

However when I restart they don't open & are no longer listed in Accounts - Login Items.
 
The permissions on your login items plist have probably gotten messed up. Try dragging ~/Library/Preferences/com.apple.loginitems.plist to the Desktop, then logging out and back in. Then open System Preferences and try adding your items to the login items again.
