My year-old 5g Touch's screen finally shattered after its last fall. I'm deciding whether to pay Apple or someone to replace the screen or to put that money toward a refurb. Any ideas on screen replacement cost?

Or, if I just keep it as is, might the screen eventually blacken and die?

Or, might shards of glass come off and slice an artery?

Find out the replacement cost and find out a screen replacement cost. Google will be your best friend with this one.
 
A colleague used a broken iPhone 4 for two years; no problem. Beside the cosmetic impact.

To protect your artery you could add a protective foil on the screen; that would keep the glass together.

Unfortunately cracked glass tends to crack even further. I suggest you use the iPod service that swordfish5736 linked to. The replacement is a refurbished unit.
 
I just went to the Apple store and paid $149 for the replacement. My genius told me I got a new unit, not a refurb. He said they didn't have refurbs in stock. Sweet!

Few thing you should definitely do:
1)Get a protective case like an otter box or a lifeproof. A case with a lip in it.
2) get a screen protector. Plastic or glass.
