Looking for clarification on background refresh to see if I am having an issue..

I am trying to understand frequency of background refresh. Have this feature turned on for every app in my dock both native and 3rd party and not one of them are ever updated with current info when I open the watch dock. There are some apps I that dont use for days and when i bring up the watch dock and I flip thru the dock apps every one of them has app data from the last time I looked at it whether its 6 hours ago or 3 days ago. This includes homekit, native stock app, and various other news and weather app. Only when I click on the specific app in that dock does it update after giving me the busy icon. Have latest ios and watch os on an series 2. As a sidenote my complications seems to update just fine. Is this just normal behavior? Though the point was to have immediate current live data as soon as the watch dock opens.
