Looking for graphics card information

Hello! As many of you know, I am the main admin for the Apple Wiki. Recently, other than trying to kickstart various pages, I've been working on the video cards section of the Wiki. My goal to this post is to gather information for the page. I know we have many members here with upgraded, modded, and flashed cards. We are looking for any card aside from the 6200. Preferably your reply would be formatted like so

ROM (no or yes, if so provide download):

Taped pins (no or yes, if so what pin #s):

VRAM:

Machine proven to work with (Model name and model #):

Version of OS X:

OpenMark score (if applicable):

Some of this info could be collected from places like themacelite.wikidot.com and cubeowner.com. I don't know if other models have lists of compatible graphics cards.
