UPDATE: I found Wufoo, I forgot about this awesome form site. I no longer need help. Thanks though.

My work uses Filemaker Pro for a local database for the rental of production equipment. I found a nice web service for reserving time in the editing suites. Up until now we have been using pencil and paper to book reservations. With our recent expansion to 24 suites it has gotten cumbersome. We also just got a new studio built that will need a reservation system. Luckily I found supersaas.com and they seem to have an awesome solution that should save us time and money. It is a lot like the Apple Store's Genius Bar reservation system.

What we need now is a similarly easy to use web app that can handle the equipment rental process. Does anyone know of a browser based service like this?

If not, we could still use Filemaker Pro, along with a web based system that generates a form that the renter fills out and submits, which is then emailed to us. We can then physically check to make sure the equipment is available as a backup and email the renter back if there is an issue with their request.
