I've noticed on my new 16 Pro that the lower power mode does not pop up when it reaches 20%. Is this a bug or it's been changed?
 
I know it turns OFF at 80% charge. Not seeing anything about when it turns ON automatically - which makes me think it might depend on circumstances (battery health, usage behavior, etc)

Yes that feature was removed starting with the iPhone 14 Series. You can still make it happen using an Automation or add the Low Power mode to your Control Panel.

How to set up an automation:
  1. Open the Shortcuts app.
  2. Tap "Automation" at the bottom.
  3. Tap "Create Personal Automation".
  4. Choose "Battery Level" and set the condition to "is 20%" or less.
  5. Choose "Add Action" and search for "Set Low Power Mode".
  6. Set it to "On" and tap "Next".
  7. Turn off "Ask Before Running" and tap "Done".​

If you talking about the pop up that allows you to enable low power mode when battery hits 20%, it technically is still there just more hidden.

The warning pops up in the dynamic island and when you long press on it, it expands giving you the option to enable low power mode.

I posted about this before with screenshots:

Should be the same for 16 Pro as well.

I didn't mean turning on automatically, I meant the notification that pops up when it reaches 20%

This is what I do, automatically turns on when the battery hits 10%
