Mac does not cache external drive icon in dock

About every other time i restart the external drive icon has a question mark in the icon. However the drive is running and mounted fine. The second i click the question mark the icon pops in. So to me it just seems like the dock is not caching the icon?
 
Do you observe the same behavior if you create a new user account and/or another external drive? If so, it may be a bug in the OS.

Is the drive formatted in HFS+ or another file system? Observe with other disks formatted differently.

Also run disk utility on the drive to rule out a filesystem issue.
 
Why not just use the Devices link to the external disk, instead of creating another sidebar item for the drive?

Finder Preferences --> Devices --> Tick "External Disks" if not enabled
