As per the screenshot, I have my Mac mini M4 connected to my LG OLED TV via HDMI and there is a screen mirroring icon in the menu bar that says I am extending my screen.

Worth adding that I have a soundbar connected to the TV using eARC and the Mac mini is actually connected to the sound bar itself which has two additional HDMI ports. Reason for this is because I am out of TV HDMI ports.

The only down side so far is that I can only go as high as 60Hz via the soundbar rather than 120Hz via the TV itself. But could this also be causing the extended display icon?

EDIT - Solved (Sort of): I have sort of managed to resolve by command clicking the icon in the menu bar and dragging it out of the menu bar. This makes it disappear.

Ah what YOU have done there is assumed I have a simple setup where I can freely move things about and easily swap cables. The reality is, everything is hidden and tucked away for a nice clean setup, soundbar bolted to the wall along with the TV, cables trunked away etc. so in actual fact it IS easier to ask on a dedicated forum if someone has a solution before I start pulling my media centre apart when a simple forum reply may resolve. Not your reply though, yours was just condescending. Oh and I did google it before coming to this here dedicated forum I have been a part of for a long long time.

Nevertheless I managed to find and connect another HDMI cable eventually, direct to the TV from the Mac Mini, and as expected got the 120Hz refresh rate but the Mac still insists I am extending my display.

Oh my god… this is happening to me as well. I have literally searched for an answer but can’t find one. I’m not using a TV but my Apple TV is also plugged into my monitor. Maybe it’s a bug? I have actually messed around with the display mirroring/extending settings and it sometimes causes my Mac mini to freeze.
 
I noticed this today too. It was right after I updated to 15.2 so it’s obviously something with the latest version of macOS.

I had just turned on my Mac mini after not using it for a couple of months, noticed it, couldn't figure it out, and ended up up Erasing and Resetting because I thought I was hacked or something... smh

Please make sure that y'all report this to Apple so it hopefully gets fixed soon™.
 
So glad I’ve seen this thread. Same issue. Only one monitor on my M4 and it says it’s being mirrored to it. After spending the day trying various things, safe boot, reinstalling OS, etc and had no luck. I did attach a VGA monitor through a dongle and the screen mirroring turned off for that but it came back on when I plugged hdmi or usbc monitors back in. Sounds like it is OS 15.2 update
