Mac Pro '09 Headphone Jack not working?

For the life of me i can't get the headphone jack on my mac pro working.... is there a setting somewhere I am missing?

tried several headphones, none work... they work on other machine etc..

Yes I did, of course...

... in windows the jack works. It can play from both the line out in the back as well as the line out in the front....

I'm running windows now, but can the mac pro/osx output sound from both the headphone jack and the line out simultaneously?
