Could not find the answer to this anywhere.

Should the fans be running on a Mac Pro 2019 when the computer is in sleep? I can't hear them, but when I put my hand to the back of the device I can feel the air blowing.

the blower in the side of the case where the RAM etc is keeps running on low rpm during sleep - you'll feel the air pressure mainly from the bottom right beside the PCI slots if you're looking at the machine rear-on.
 
When I put my MP7.1 to sleep all the fans are off.

I think you'll find they're not, unless possibly you've set it to hibernate, and it's effectively powered off. In normal sleep the big three front fans for processor and PCI shut down, but the side fan that handles RAM and storage continues to run.
 
Weird, to have them running all the time. Wonder why they decided it needed to be running, especially as the 5,1 I have has all the fans off when the Mac is in sleep.

Higher performance parts, and higher spec overall. Probably also allows the machine to wake more quickly.

It requires work to get a 7,1 to go to sleep, and stay asleep. Apple likes them to be waking every hour or so, imagining that with the 5,1 - the click, the spinup of all the fans etc, it wouldn't wash.
