Hi, I have a Mac Pro A1398 EMC 2512 from ~2012 / 2013. Bought used, has always had some strange boot issues.

Was running Mojave from fresh install, working fine for a few months. (but always showing these boot issues)
Tried to install latest Mojave update and now won't boot.

Essentially it gets stuck trying all of these things around 60 - 70% loading (white load bar)

> Have tried NVRAM clear, SMC, you name it. Nothing changes

> Verbose mode gets stuck at multiple places, these are the ones I have recorded. Seems to be network related? but not sure.
  • starting optimistic dad immediately
  • controlID: 4,
  • setting BTcoex

> Recovery mode (D) local works, but after I select WIFI it spits out an error at top left about boot loader / EFI, I will have to run again to post exact error message.
Other Recovery Mode CMD-D does not work either.

> Safe Mode does work, but it takes a VERY long time to load, gets stuck around 80-90% then after 5+ minutes screen goes black then if I press keys it will log me in.

Ran Disk Utility Repair in Safe Mode, drive shows OK after that test (although I am thinking more and more it may be the drive, even though there are no bad sectors shown)

> Booting to USB Installer for Mojave, again loads around 60-70% and that is all.

Only thing I have not tried is Single User mode and running check disk, can do that later today and report findings, but not very hopeful.

Just thought someone might have ideas of what to try.

My next step is to buy a new SSD and see if that fixes issue. If that does not fix may have to take to repair shop...

I have this same problem, had a security update run when I left for work, heard the startup chime as I went out the door, got back to my mac 2 days later, now my iMac 2009 with a ssd can not get trough the boot up proces, in safe mode it worked, although very slow and the screen was flimmering a lot with some strange color bling before it stabilizes itself, try to boot into a clone, same thing is happenin, stops booting at a bout 75% !!!
