Does this mean even if my iPhone is in three pieces they'll replace, not saying it is but the message on this shipping slip is weird. Also, on all my macbook mail ins it said Limited Waranty, why does this say expensed, also I didn't use apple care plus

I have almost no idea what you are asking here. I know it has to do with a warranty. That's about all I gathered from this.
If your device is in multiple pieces it really depends on what happened with it. They will almost certainly NOT repair it under warranty unless there is a special circumstance (ie exploding iPods, etc)
