Managing Multiple Audio Outputs/Inputs

I guess I should draw out a little map, but that would take a while.

So here's what I want to do. I want to stream audio and video to Ustream. The video will come from CamTwist of my desktop, and the audio will come from my headset's mic, and the audio from my computer (Skype & computer audio). I also need an output to go to my headsets' headphones, but OS X seems to only allow one audio output at once.

Any ideas? I'm getting so lost...
