I often use my MacBook Pro in bed, and lately when doing this (only started a few days ago) my fan starts spinning really loud and then makes this weird grinding noise, then it goes back to normal. Is this something I should be worried about?

Yes. Your fans are toast, are stopping suddenly and restarting. Replace them. Had the very same sound when my left fan crapped out.

Is this pretty urgent? Like how quickly could it do damage (and how much damage) if I don't replace them yet?

You have a slim chances of frying components if the fans aren't able to cool them, most have a fail safe and would shut the computer down.

Personally, the noise in itself would be enough incentive for me to crack the machine open and change them.
