MBP goes to sleep as soon as I wake it up

Google couldn't deliver the answers so chose to ask here.

This happened about 4 times now, just happened now as well - Whenever I wake my MBP from sleep, two-three seconds on the screen goes blank. It has no response for few seconds, then I press something and it asks me for login as if it was just sleeping (when I just logged in few seconds ago!!!)

For further information,
- System Preferences: Display sleep 10mins, Computer sleep 10mins when with power (my MBP is always powered all the time)
- Late 2011 MBP 13 inch model
- No scheduled sleeps in settings whatseover.

Google "SMC reset"

then do a pram reset
