When I insert a memory card into the Mac via a card reader, half the time nothing happens. I have to remove the card and re-insert it, then it will appear in the Finder.

On all my Macs for the past several years I have had this problem, whether it be the built-in SD card reader of the mid-2010 MacBook Pro, the card reader in my Dell 2406 monitor on my mid-2009 Mac Pro, or any variety of USB card readers that I've used with all my Macs.

It has been happening since 10.5 or perhaps even earlier.

I wonder if it's something to do with my system configuration or installed kexts, etc.?

On Console this happens when it does mount:

2/15/11 1:53:23 PM fseventsd[64] check_vol_last_mod_time:XXX failed to get mount time (25; &mount_time == 0x10057f8b8)
2/15/11 1:53:23 PM fseventsd[64] log dir: /Volumes/NO NAME/.fseventsd getting new uuid: 44BA9755-F903-46A2-9255-A1CD1C179D3B
2/15/11 1:53:23 PM PGP Engine[265] Processing disk1
2/15/11 1:53:41 PM kernel AppleUSBMultitouchDriver::validateChecksum - 512-byte packet checksum is incorrect (expected 0x9076, checksum bytes were 0x1c1c)

When I eject it:

2/15/11 1:55:07 PM fseventsd[64] disk logger: failed to open output file /Volumes/NO NAME/.fseventsd/6365734de4e5bbf7 (No such file or directory). mount point /Volumes/NO NAME/.fseventsd
2/15/11 1:55:07 PM fseventsd[64] failed to unlink old log file /Volumes/NO NAME/.fseventsd/6365734de4e5bbf7 (No such file or directory)
2/15/11 1:55:07 PM fseventsd[64] unmounting: failed to remove log dir /Volumes/NO NAME/.fseventsd (No such file or directory)

Then I reinsert & it mounts:

2/15/11 1:55:57 PM fseventsd[64] could not open <</Volumes/NO NAME/.fseventsd/fseventsd-uuid>> (No such file or directory)
2/15/11 1:55:57 PM fseventsd[64] log dir: /Volumes/NO NAME/.fseventsd getting new uuid: FFF67711-3628-4D31-A175-8CC4531BE2E2
2/15/11 1:55:57 PM PGP Engine[265] Processing disk1

Sometimes I eject it and NOTHING happens on Console.

When I insert it but it doesn't mount, NOTHING shows up in console.

Sometimes it mounts just fine several times in a row. It seems to always mount fine after one time of not mounting. So it's intermittent, but unpredictably so. Affects all my computers.
