Mid-2012 Macbook Pro sleep issues

Hello,

I have been having this issue for a while now. Whenever I try to wake up my Macbook pro after it has been in sleep mode for a couple of hours, the computer freezes for a good 45 seconds. It also takes about 15 seconds for the time to switch from the time on which I put the computer on sleep to the time on which I open it.
