I've been using this one for about a year and a half with no problems. Any brand will do.

Yes use the monoprice one. I've never had any issues and even if you need to replace it, you can buy 5 of them for the same price as one apple branded one....

If you are looking to hook up a DVI monitor at home, as opposed to using it to connect to random projectors when presenting etc, I would strongly recommend buying a MDP to DVI cable, as there is no chunky adapter six inches from the port on your laptop. I bought one a year ago on ebay for $10 and it works great.
