Modem "Disconnecting" Continual Loop...

If I log on, or wake from sleep and immediately go to connect to the net with my internal modem, sometimes it says connecting, but then goes to "disconnecting" and stays in an unstoppable loop, to the point where I have to restart the machine to be able to try and connect again..

Any help?
