I have my MacBook Pro hooked up to a monitor on my computer desk, never had any problems.

But lately when I leave the computer for a while and I come back to it the monitor does not come back unless I open the MBP, unplug the display from the MBP and tell them to plug it back in.

Its very annoying and I have no clue what to do to fix it. I have disabled power saver and screen saver, yet it still does it.

Any clues?

i assume you mean clamshell mode?

could it be your mouse or keyboard is losing connection? I think this can effect clamshell mode

Nope, its definitely not keyboard or mouse. Because after it goes out, it doesn't come back until I Unplug and plug back in the monitor.
