Basically the title. All apps are jumbled now. Anyone else also having this?
 
i have a leftover "Install MacOS Monterey" gray icon with a question mark in the Launchpad?! How to get rid of it, i can't see the installer anymore in Applications

If you do an erase all contents, it removes it. Make sure you do a backup first because the Data partition will go bye bye.
 
The icon can be gotten rid of by first clicking on it (you'll notice after you do this it'll show as a '?' icon), then going back into Launchpad, clicking and holding on the icon to go into 'wiggle' mode, and then deleting it by clicking the 'x' that appears.

As for the apps being jumbled up, I can relate. My app arrangement has been changed so all the default apps are on top, and then all my third party apps are strewn around in a random order below. My folders are intact though.

I'll rearrange them at some point, but I'm trying to unlearn the habit of going into Launchpad constantly in favour of using Spotlight. I do this because I don't think Launchpad will be around in future versions of macOS given how little attention Apple give it!

These have both been bugs for many years, over many major macOS versions. The Launchpad extra Installer icon is fixed like adamlbiscuit says.

The icon jumble bug is fixed by typing:

defaults write com.apple.dock ResetLaunchPad -bool true; killall Dock

In the terminal.

I saw the same thing after installing Monterey. Clicking the icon does not open anything. And there is no install macOS Monterey app in the application folder. I first thought this occurred because my first download of Monterey through software update was unsuccessful (stuck forever) and I restarted the download. Click and hold the icon, a "X" will appear on the icon. Click the "X" will get rid of "Install macOS Monterey" grey icon.
