I am not sure if this is a hardware issue or something that can be tweaked in OS X. I have a PowerBook G4.
Sometimes it seems like the mouse stalls and will not perform a clicking task. - For example, say I want to open Utilities in the dock next to the trash, but I accidently click on Applications. Instead of releasing so that Applications opens up, I hold down the mouse button. Then the Applications list expands. I then move the mouse off of Applications and click on Utilities. I keep clicking and it will not open Utilities. I wait and bit and try clicking and it still will not open. I have to move the mouse off the dock then back to open Utilities.
I have also had problems like this with trying to close tabs in my browers, or files and folders.
It seems that if I accidently miss the close "x" or folder and I do multiple clicks, there is some sort of "click protection" or something so there are no accidental closures etc.
It is not my mouse button, because the same thing happens with the trackpad clicking. So I am not sure if this is just the PowerBook hardware, or if there is something that can be tweaked in OS X to fix this.
