Yup. :) This comes directly from the Messenger:Mac help file:
Microsoft:Mac help said:
1. On the Edit menu, click Emoticons.
2. In the Emoticons dialog box, click [the plus sign].
3. To locate and select the picture you want to use as an emoticon, click Select File.
4. Type a unique shortcut that, when typed in the conversation window, will add the emoticon to your message.
5. Type a name for your emoticon. (Optional)
6. Click OK.

You can see the help by clicking on "Help -> Help Topics" in Messenger. Your web browser will open and connect to msn.com to show you the Messenger:Mac help.. From the table of contents there, I chose "Customizing Messenger", then "Create your own emoticons".
