it's happened more than just a few times now. On my iPhone 6 iOS 8.4 I'll plug in my EarPods and start Apple Music. Volume is lower than the last time even thought the volume slider is in the exact same position as before.

The only thing that really fixes this is to restart the iPhone. Then it's back to normal volume. What gives? Maybe iOS 9 has a fix? Very curious bug.
 
I found the culprit - Facebook. I was listening to Apple Music and happened to open the Facebook app. When I did the volume decreased by 20% minimum. Wonder if its because of the auto-video-background feature of Facebook?
 
i've issued that one few times..as you said..it's facebook bug, maybe you suddenly hit videos and starts immediately, and then music volume decreases automatically, disable auto playin videos.

Didn't think about that. I disabled auto=play. See what happens now.
