I have a new 6S+ and it's fully updated to iOS 9.0.1 and my alarm did not go off this morning so I got up an hour late. This happened once before on iOS 9.0 but I thought they fixed it with this update...
 
My alarm went off this morning, but not to the song I usually have. I had to reset all of my "song" alarms. I'm not at all sure the ringtone they used would have woken me up if I hadn't already been up.

*grumble*when are they going to let you pick "random song from a playlist" as an alarm*grumble*

I'm pretty sure my alarm went off, although it's a bit fuzzy since I think I was awake enough to turn it off but nodded off asap again.

Definitely a case of volume set too low and me being desensitised to the previously-much-too-loud vibration on my bedside.

I was 2 whole hours late to work as a result.
 
Mine did not transfer from my iCloud update but I only work up 4 mins past my usual time and checked my phone and jumped out of bed. Does that stuff usually transfer from iCloud back ups?
 
I'm running iOS 9.1 Beta 2 and I think an erratic alarm is one of the fixes.

Ah, found it:

  • Fixes an issue where sometimes alarms and timers could fail to play
 
make sure the volume wasn't all the way down as even if it's on mute there has to be some volume.
 
The only thing I've noticed is that if I assign a song from Apple Music as my alarm then it won't work. But if I assign a song from my actual library that I've purchased and downloaded then it will work.

Apple might not trust your choice of music (see: Potential effect of Nickelback and sleeping :p)
