Just as the title suggests, I am wondering how this use case is handled. Both my iPhone 4 and the iPad would be logged into iTunes with the same account. How do I facetime to my family from my iPhone? How would it work?

Create a separate Apple ID for each device. You still log on to the shared account for iTunes and apps, but for FaceTime you should be logged in as your personal name.

also, you can have multiple devices logged into one name. So for me if someone tries to FaceTime it will ring both my Mac and my iPad.

I have an iPhone 4 and an iPad 2. I have facetimed myself. The iPad should be based on the email address and the iPhone based off the phone number.

BTW, the feedback is terrible when the iPad and iPhone were right next to each other. It was sort of like circus mirrors, though, with 4 pictures of just me going.

so in FaceTime do you configure an email? What happens when multiple people share it - do they have to change the setting each time?
