OK, so I tried downloading a free game (brick classic i think) and it just stayed "loading" forever. So I tried installing again, so on my screen I have the original "loading" icon and now the new "installing" icon, for the same app.

The 2nd attempt installs fine, so I delete it to clear the icon and start from scratch. However, now I have a phantom icon that won't go away. You can't see it, but the space is taken up (as in, when I try to rearrange icons, I can't place them where this icon was supposed to be).

When I do try to put a legitimate icon in this space, very briefly the game's icon comes forward and then disappears into the background again...

HOW THE HELL DO I REMOVE THIS ICON/APP?!?

I had the same thing happen to me. I tried restarting, and that didn't fix it, but then I just synced the phone, and it fixed it!

This happened to mine but some how after deleting one of the icons the other "appeared" and then I could delete that too.
