Need help with creating a form

Hi,

I've been asked to put a form on a website I've created that would allow people to sign up and pay the entry fee for a charity walk. Upon completion of the form, ideally an email would be sent to an email address with the details so they could then see who's signed up.

Ideally, I'd like to use pay pal as the payment method.

What would be the best way to go about this?
