need help with those annoying dip switch video adapters

I have and old blue&white rev. 2 g3 and a 15" applevision moniter I also have the required adapter but alfter fiddleing with those switches and searching online i cant find a freaking table to show what settings are required for 1024x768 all i have found are weird resulutions like 832x612 and such
