Hey,

I am trying to create a fillable form, with form fields, using Neo Office. I have scanned and attached the image a sample, to give an idea of what I am trying to accomplish.

I need to create a form with the blank lines and the reference text ("date" , "address", etc. - as sampled in the form) underneath the lines. I was able to get the line and wrap it around the text. However I am having trouble with two things.

1. Inserting the reference text underneath the blank line.
2. Changing the spacing between a few selected lines. When, in Neo Office, I highlight two lines and change the spacing using format->paragraph-> line spacing, the word processor changes the spacing between all of the lines in the document. I need to change the spacing between a few selected lines.

I also have pages on my mac, but as far as I know, pages doesn't offer form field functionality.

All help will be appreciated.
