They very rarely do. I don't even use any other than the defaults other than for alarms and timers. Then there're those who always keep the device on silent.
Regardless you can simply choose something different for a ringtone rather than use Apple's stock ones.

You can make some pretty banging ringtones with Garage Band and use those.
In the past, new ringtones/alert tones have (if memory serves) generally appeared in the public release, rather than the betas (since there's no real need for developers to test those)
True.
But the biggest overhaul was with iOS 7, when they introduced the current ringtones...
Will be a decade since the last refresh next year. Could do with a few new ones

I just download the ringtones for a S22, convert them to M4r and then add them to my phone.
