iTunes Home Sharing works great...until my 2009 MBP sleeps. Then the "shared" stuff disappears on my iphone.

Yes, I enabled "wake for network access" in energy saver prefs. And made sure my home wifi is topmost in "Network" preferred networks

Note that my MBP always has the clamshell shut (I use an external display).

Is it supposed to work? Sleep works because it turns the hard disks off, so there's way the computer can access the media. Furthermore, if its sleeping, the wifi radio is off, so it can't stream the song to your iphone either. The purpose of home sharing (I'm pretty sure) is when the computer is on. It sucks if you have a laptop, but I guess it works better with a desktop.

Also with wake for network access, make sure you're connected to ethernet.
