iPhone 13 no notification on call, message or email

I have an iPhone 13, an apple watch 3, iPad and mac 2011 pro. Phone & iPad running 15.1.1, watch 8.1, pro and AT&T provider. I have turned off "Silence Unknown Callers". The phone doesn't ring if a caller I do not have in contacts, calls. It will ring and show it's spam, telemarketing or unknown numbers.

I also do not get message nor emails notifications when delivered.

This true for phone, watch, and I ipad.

Have I got settings wrong or AT&T?

Could you have a focus mode set to do not disturb?

If I have a Focus Mode turned on and get a call or text from outside the allowed contacts, my phone does not ring or alert, which is the correct action. @ericwn is on the right track I think about a Focus Mode being on you may not be aware of.
