No signal or loose connection when connecting an iPad to an external display?

Hi, I have an iPad 12.9" 2021 with M1. Quite often when it is connected to a TV via Apple's USB-C Digital AV Multiport Adapter , I get loose connection and the TV reports that there is no HDMI signal. What is the cause of this? Is the lost in connection due to the weight of the adapter and the HDMI cable? Putting the entire thing on a desk does not help. Is this a common problem among iPad? What can we do to prevent this from happening?

I also have the same issue with iPad 12.9" 2017.

For the past two hours all of a sudden connection to external display does not work. There is no blue indicator on the top right which indicates connection to external display. Changed two different cables. Changed from Apple’s adapter to Caldigit Element Hub->USB-C to HDMi adapter to TV. No blue indicator on screen, no signal to TV. Is the iPad failing?
