I have searched endlessly for a fix. I know this has been addressed, but I have not seen my exact problem.
I just purchased a Mini Display to HDMI w/Audio Cable from MonoPrice.
The video works fine, I am able to see and select my TV from the Sound Output in System Prefs but I still get no audio. I can click through the list (headphones, built-in and my 2 Airplay devices) and they all work while the video plays, I just never get the audio when I select my TV.
Any ideas?

This issue can sometimes be resolved by opening the Audio MIDI Setup application (located in Applications/Utilities) and changing the output setting to 44 KHz (44100.0 Hz).

1. Open the Finder
2. Applications> Utilities> Audio MIDI Setup
3. Click the drop down menu after Format and choose any of the options
