Not all icons in theme show up

I'm using the Glasklart HD theme on my 4.1 iPhone 4 and not all of the icons that are in the theme are showing up. I SSHed into my phone and saw that the Sleep Cycle app and ESPN Scorecenter app have icons, but the default icon shows up on my iPhone. Also for the ones that don't have an icon, how would I go about importing them from the request thread on macthemes?

Make sure they have the correct spelling and capitalization, they won't appear correctly if it looks like this MINIGORE.png

com.espn.ScoreCenter is the folder inside the bundle then icon.png is the image under the folder. com.lexwarelabs.goodmorning and icon.png along with icon@2x.png for Sleep Cycle

For com.espn.ScoreCenter, it needs Retina icon named Icon@2x.png

For com.lexwarelabs.goodmorning, try naming the retina icon iTunesArtwork.png

As for creating new icons, or installing icons look at the first post in the macthemes thread.
