This has been killing me for a while and basically makes Notes useless for me on my phone. On my two Macs and iPad notes show up fine. But on my iPhone (4S on latest OS) it constantly duplicates entries. Even worse than that, it doesn't seem to sync consistently. (Look at the note "Things to buy". There are two of them, 2 days apart...)

I also have no local notes, just the "All iCloud" notes.

Anyone have this happen to them and/or know a solution?

I managed a solution. I had to manually delete all the 'duplicates'. For some reason they wouldn't show up on other devices, and would keep updating themselves. All seems working ok now.
 
I had this problem between my iPhone and iMac. Stopping the iMac from going into sleep mode resolved my issues.
 
I'm on iPhone5 and just saw that one simple note in my notes app has spawned over 20,000 entries. There is no way to delete them using the UI. I do sync with iCloud but this is more like a malware attack. Only me?
 
Nope.

I just have to periodically delete the spawned extras.

I'm wondering why this isn't a more prevalent issue.

Devices that I sync Notes with:
2008 iMac with 10.8.4
2010 MaBook pro with 10.8.4
iPhone 4s with 6.1.3
iPad2 with 6.1.3
 
Problem for me also

I also have this problem on my iPhone 4s.....but they are not not duplicating on my iPad Mini.Would love to find out a fix!
 
Try this method, same concept but with notes and not Safari.

I've never seen duplicated Safari bookmarks and now Notes.
