I have a document stored in iCloud Drive. I open this document in Word on my Mac, and in Word on my iPad. I always make sure to save and close out of the document before opening it on a different device.

Word on the iPad keeps randomly saving my changes to a local copy of the file, on my iPad. I'll navigate to Files, open the app in iCloud Drive (this launches Word), type for a while, hit save, exit out of the document -- only to find that the file has been saved to my iPad, and changes have not been uploaded to iCloud Drive. Ridiculously bad integration.

Anybody else experiencing this? Any tips? (Other than just not using Word on the iPad?)

Use OneDrive instead. You get more storage for free than iCloud, and the paid tiers are better value, and include full Office 365 on iOS, Mac, and Windows.
