I have the OS on an SSD and my home folder on a separate HDD (iMac). I am looking to make one bootable image to be installed on my MBP (one SSD) and as a backup as well.

Has anyone done that before? How?

I don't think can happen because in effect that won't be a cloned disk image will it. If you want that, you'll need to go the traditional back up route.

What you could do is to make a bootable clone of the SSD, mount it and then copy the home folder to it at a carefully selected location.

Personally I'd rather make a normal install on the MBP, and unless I'm migrating my setup to the other computer I wouldn't use a duplicated home folder on it.
