Ordering through at&t/apple

i've been an iPhone user since the beginning but have never ordered one from my carrier or directly via Apple. I have an iphone to trade in but could somebody let me know what the process is? Does the phone come ready to use and then you just send in your old phone? Does it work the same if you get it from AT&T or Apple? Sorry, I know this is pretty basic but I've just never done it before!
