Ordering Verizon iPhone without subsidy

So I've got a friend who has a Verizon account that's ineligible for an upgrade, and they've offered to let me order a phone through that account. Couple questions:

1) The apple eligibility checker shows the full price, but it also says "2 year contract" -- do they really make you extend the contract to 2 years even if they're not subsidizing anything? (image below)

2) Can I order with a credit card that has a different name and address than the wireless account?
