Pages 5.5.2 — 2-Page Spread?

I'm trying to send the cover spread (front & back) for a book to a publisher. They ask for the full spread with spine, etc. I can't find anywhere on pages that can give me that template. Any ideas will help.

One of the many features removed in Pages 5.0. I kept a copy of the old Pages so I could do book layouts.
