Im looking for a PDF application that will allow me to complete PDF forms that contain text fields on the ipad

TakeNotes kind of has this functionality. It allows you to import a PDF into it, overlay text boxes on the pages, type into them and then re-export the document as a PDF. Take a look at their site, they have a demo video IIRC which demonstrates this in action.
