PDF Form App

Ok so here is my deal:
For my job I have a paper form where I collect information. Is there an app where I can basically create a form template and enter information in to various fields? Then export as a pdf or something of the sort? I am trying to eliminate hauling around a bunch of papers and getting fresh copies from the office if I can just have a form on my ipad.

I would use Goodreader. you could scan the paperwork into a pdf format and then load it into Goodreader. within Goodreader you can annotate the pdf and save it as an annotated copy. that way the original pdf is always blank.
