After a year of battling Personal Hotspot on a daily basis, and hoping that each incremental update for with iOS or OS X would fiX the problem, I can finally say that Apple seems to have fixed the issues I used to experience.
The company that I work for has slow 1mbps internet connection due to my boss being loyal to the local internet company and refusing to get Comcast , so I use my phone for internet access on my computer while I am here. Plus I have 30GB of data with AT&T, so there's plenty to spare.
Every day when I try to connect, it takes me at least a minute or two, and generally 3-4 tries for Hotspot to connect to my MBP.
Eventually I discovered, if I click the Wifi Icon, wait for hotspot to show up, turn Wifi off, turn wifi back on, turn hotspot off on my phone, turn hotspot back on on my phone it would usually connect. I've always been able to get it to connect, just never been able to have it connect consistently on the first try.
But ever since iOS 9 and El Capitan, it's connect every time on the first try. Fingers crossed that it continues!
