Okay, so this is a new one for me - with MacOS 12.2.1 (on 2017 Intel Air, 1.8 / i5)

I open an image in Photos, Edit it (including Retouch) > Done > open Library > highlight pic > File > Export > Export 1 Image > allow it to go with default settings > Export. I go to the location of the file and the image is not the edited one, but appears to be the original.

I've used the same process in the past with no issue whatsoever. I don't do this super often, so I can't confirm if problem seems to have appeared with most recent OS 'update' or with 12.2 (27 Jan). Anyone else? Thoughts?
 
option / alt when opening Photos found a number of .photoslibrary folders - some dating back to the first boot of this computer. Not sure if anything there was the problem, but sought them out and deleted them, rebooted and opened Photos and allowed it to create a new .photoslibrary folder.

Tried a random photo > Retouch > Done. It showed up as retouched in Library - but when exported out, it was not. Same as original.
Tried same pic with other effects - crop, color enhance, greyscale. All exported out without issue. As they should be.
So it seems my Retouch is buggered. Not sure why.
 
This appears to be a bug. I see the same behavior when I export by selecting the thumbnail.

It seems to export with the retouch if the photo is opened first and then exported from there. Or the thumbnail from the library can be dragged to the Desktop (or some other folder in Finder), and that retains the retouch too.

Ah, well - modified good! Good that it's not just me, modified that this bug exists at all.

Thanks too for the solution! Very helpful! I guess we'll see if the next MacOS 'update' overwrites this problem.

I posted the issue on Apple Communities, and I rather got the reaction I expect from there - another user blaming me for using the Photos app not in its intended way, and that's why the problem.
'Twas always thus there - decades of the community's more...ahem, 'enthusiastic' fanbase live there, defending Apple against issues found by users. Apple's first defence force, until an issue becomes so big (or is noted in the Media) that Apple are forced to deal with it. But the Macolytes persist, rather than offer solutions.
 
Interesting Addendum to this - another user elsewhere acknowledged same issue, but with another layer - as it were -

Confirmed highlighted thumbnail that had Retouch done > Export would not be the fixed photo. They then took the original pic, added Sepia and Retouch > Saved it > selected thumbnail > Export. Exported image included Sepia but not the Retouch.

Confirmed that indeed an opened image in Photos (regardless of stuff done, including Retouch) would then Export the image with everything, as would the drag of the thumbnail to, say, the Desktop.

Something's definitely buggered.
 
Definitely a bug with MacOSMonterey, just had the same problem. The problem is using the export function and it only negates the retouching, but does export the cropping and any color adjustments - very weird. However I did discover if I just do a copy and paste or a drag and drop the retouching is retained. So for now I will not use the export feature for photos I have retouched. The export feature (when it work properly) was nice in that you could select the compression (High, maximum, etc.)

Just tried it in Ventura 13.2 and it appears to be fixed.
Good news - for those able to use Ventura.

For me on a 2017 Air (sold by Apple well into 2019), it's not an option. And of course, Apple's now lost any interest in fixing Monterey bugs.
