Photos in the wrong album

My iPhone and iPad is acting weird. Photos will be in the wrong album. I tried uncheck the photo in iTunes and delete the iPod cache folders then check the photo in iTunes again but it's still happening. Anyone knows what the problem is?

Same problem and tried same things... WTF?

Rebuilt my iPhoto library, and ended up with more pics messed up in iPhoto.. something is jacked.
