Photoshop cs3 prints 4 shades to dark

I am printing cs3 to epson4800 prints. have calibrated believe
I have the right printer settings prints coming out 4 shades darker than
screen Any ideas?

Are you soft proofing?
Is your monitor set to 2.2 gamma and fully calibrated?
Is your printer calibrated?
Are you using the correct paper and ink profile?
You need to answer all these questions first.
