PHP Form Wizard Equivalent for Mac?

Hi All,
I need to generate a large HTML form (with PHP) validation that sends the results via email. I used to use PHP Form Wizard on the PC, but it's PC only. Can anyone recommend software for rapid development of HTML/PHP forms?

(And yes, I could code it all by hand but it will take a long time because it's a long form).

See if JotForm would work for you. I use it all the time for some of my web customers. It's easy to use and not too expensive.
